Provider Questions — Checklist Before You Commit

1. How many implants are included? What brand? Is it 4 or more?
2. What does the quoted price include: surgery, temporary, final, imaging, follow-ups?
3. What type is the final: zirconia or acrylic? What is upgrade cost?
4. Is IV sedation included or extra? What type of sedation is used?
5. Are extractions and bone grafts included? If not, what is typical cost?
6. Is there a temporary fixed set same day? How long until final?
7. What is the warranty or remake policy?
8. Can I see before/after of your own cases (not stock)?
9. Who does the surgery and who makes the teeth? Same office or lab?
10. What is total cost for 1 arch and for full mouth (2 arches), written?
